Insurance Information

Please understand that insurance policies are a contract between the policyholder and the insurance company.  Our office is not a party to that contract.

The filing of insurance claims is a courtesy that we gladly extend to our patients.  You will be required to pay an estimated portion of the bill at the time services are rendered.  This estimated co-payment will vary depending on your insurance benefits, including your annual maximum coverage, annual deductibles, and the percent at which your insurance covers each procedure.

This is an estimate and not a guarantee of payment by your insurance company.  Any unpaid professional fees are due and payable in full by you. We will cooperate in any way we can to help you obtain maximum benefits, but we suggest you become familiar with your plan.

Patients coming in for a consultation prior to surgery will be provided with a comprehensive treatment plan outlining their insurance benefits, our fees, and the anticipated co-payment due the day of surgery.

Following is a list of plans for which we are preferred (in-network) providers.  Some plans (Aetna for example), may offer the same coverage whether or not you stay in-network. You may contact the customer service number located on your insurance card and ask them to compare in-network and out of network benefits.  If you prefer, we would be happy to call and obtain that information for you.
